The disciplinary notices are as follows:
David T. Davis MCSI
As a result of publication of a Final Notice from the FCA regarding disciplinary action taken by the FCA against Mr Davis, the Institute became aware that he appeared to be in breach of membership regulations. He was invited to appear before a Disciplinary Panel for breaching membership regulations relating to the imposition of a penalty upon him by a regulatory body and for failing to so advise the Institute.
Having taken into account the nature of the cause for action by the regulator and the penalty imposed by the regulator on Mr Davis, the Disciplinary Panel ordered that the member be reprimanded.
Verdana Parikh Chartered, FCSI
As a result of publication of a Final Notice from the FCA regarding disciplinary action taken by the regulator against Mrs Parikh, the Institute became aware that she appeared to be in breach of membership regulations. She was invited to appear before a disciplinary panel for breaching membership regulations relating to the imposition of a penalty upon her by a regulatory body and for failing to so advise the Institute.
Having taken into account the nature of the cause for action by the regulator and the penalty imposed by the regulator on Mrs Parikh, the Disciplinary Panel ordered that the member be reprimanded.
Shane Balkham ACSI
As a result of receiving an application for a Statement of Professional Standing (SPS) made by member Shane Balkham, which was supported by documentation purporting to confirm that he had completed the gap-fill training necessary to meet the SPS requirements, the Institute believed that the documentation submitted was false.
Mr Balkham was invited to appear before a Disciplinary Panel for behaviour considered to be in breach of the CISI Code of Conduct and consequently membership regulations. The case against Mr Balkham was considered proven and the panel ordered that he should be expelled from the Institute for a period of two years.
In accordance with the CISI membership regulations, Mr Balkham sought to appeal against the decision of the panel and this was accepted. At the subsequent appeal hearing, a disciplinary panel accepted that the case against Mr Balkham had been proven, but taking into account further evidence provided to the panel, the sanction imposed on him was reduced to suspension from membership of the CISI for a period of 12 months.
Neil Joy MCSI
As a result of being found guilty of the criminal offence of driving with excess alcohol, Mr Joy was deemed to be in breach of the CISI Code of Conduct and membership regulations and was invited to appear before a CISI Disciplinary Panel.
The Panel found the case against Mr Joy to be proven and he was required to take the CISI IntegrityMatters test.
Simon Stodart
Following notification that Mr Stodart had been dismissed from his job for gross misconduct, resulting in breaches of the CISI Code of conduct and membership regulations, he was invited to appear before a CISI Disciplinary Panel.
The Disciplinary Panel found the case against Mr Stodart to be proven and determined that he should be permanently expelled from the Institute.
